New Family Enrollment Forms

New families, welcome! To complete your enrollment, you can either:

  1. Download and print the forms: Fill them out by hand and submit them physically.

  2. Start online, then print: Fill out the forms online, print them, and submit them physically.

Existing Family Enrollment Confirmation

Enrollment confirmation and schedule preferences must be submitted annually by March. You can choose either of these convenient methods:

  • Download and print the forms: Fill them out and submit them in the yellow folder at school.

  • Fill out and sign digitally: No need to print! Complete the forms online and submit them electronically.
